Eczema, food allergy, asthma, and hayfever aren't four separate bad-luck events. For many people, they're chapters in the same story — and the story often starts at the skin.
The sequence has a name: the atopic march. It typically opens with eczema in infancy, followed by food allergy around age one or two, then asthma in early school years, and finally allergic rhinitis — hayfever — in late childhood or adolescence. Not every atopic individual moves through every chapter, and the order isn't perfectly fixed. But having one condition meaningfully raises the odds of the next.
The thread running through all of them is a leaky skin barrier, often tied to mutations in a protein called filaggrin. When the skin's surface doesn't seal properly, airborne and food allergens slip through — not via the lungs or gut as intended, but directly through the skin. That's an unusual route, and the immune system treats it as suspicious. It responds with a Th2-skewed immune response: elevated IgE, eosinophils, and the inflammatory signals IL-4 and IL-13. Once that immune bias is established, the body is primed to overreact — not just at the skin, but at the airways and nasal lining too. Longitudinal birth cohort studies put the risk of developing asthma at roughly 50% for children with significant eczema, with hayfever following a similar pattern.
The practical upshot is real. Treating eczema aggressively early — keeping the skin barrier intact with emollients and managing flares promptly — appears to reduce the likelihood of later respiratory allergic disease, according to emerging evidence. Early introduction of common food allergens like peanut and egg, under medical guidance, is now standard practice precisely because it routes sensitisation through the gut — the intended door — rather than the skin.

What is the atopic march and how does childhood eczema predict later hayfever and asthma?
The atopic march describes a sequential progression of allergic diseases beginning with atopic dermatitis (eczema) in infancy, followed by allergic rhinitis (hay fever) and asthma in later childhood or adulthood. Longitudinal birth cohort studies consistently demonstrate that childhood eczema significantly predicts subsequent development of atopic asthma and hay fever, with affected children carrying approximately a 50% risk of developing asthma. However, the march is not strictly linear or universal—co-occurrence, variable sequencing, and non-progression are common, and eczema predicts atopic but not non-atopic asthma specifically.
How it works
Filaggrin gene mutations and skin barrier dysfunction in eczema allow epicutaneous allergen penetration and Staphylococcus aureus colonization, driving Th2-skewed immune responses (IL-4, IL-13, IgE production, eosinophilia) that systemically predispose to nasal mucosal inflammation (allergic rhinitis) and airway hyperreactivity (asthma). Importantly, allergic sensitization appears to be a critical intermediary step, as sensitized children with atopic dermatitis show substantially stronger progression to respiratory atopic disease than non-sensitized children.
